Request for accessibility provision
Request details:
- provide your first name, surname, address, telephone number, and e-mail address
- indicate the form of contact that would facilitate handling your matter with the Office regarding architectural or information-communication accessibility
- specify your preferred method of contact
- indicate how we should provide accessibility for you
Methods of submitting the request:
- send the request to the address: Statistical Office in Łódź, ul. Suwalska 29, 93-176 Łódź
- leave the request at the Office’s Registry (ul. Suwalska 29, 93-176 Łódź)

- send a fax to: 42 683 90 01
- send an SMS or call: 453 062 290
- visit the Office and meet with staff between 8:00 a.m. and 3:00 p.m.
Timeframes for processing the request:
- no later than 14 days from the date of submission
- up to two months in complex cases (you will be informed of this and a new deadline will be set)
We provide support from other persons as well as technical support.
Where and when to file a complaint?
If we fail to provide accessibility within the designated timeframe, you have the right to file a complaint regarding the lack of accessibility. You may submit a complaint to the President of the Management Board of PFRON within 30 days.
